How to fill in your blood pressure log

  1. Sit and rest for 5 minutes before you measure, back supported, feet flat, arm resting at heart level. See how to measure at home.
  2. Write the time you measured.
  3. Write both numbers in the box: the top number (systolic) before the printed slash, the bottom number (diastolic) after it.
  4. Take a second reading a minute later and write it in the next box.
  5. Write your pulse from the first reading.
  6. Add a note when something could explain a number: coffee, exercise, stress, pain, a missed dose.
  7. At the end of the week, fill in the average row. The average calculator does the sums.
Very high reading? If your blood pressure is above 180/120, wait a minute and measure again. If it is still that high, contact your doctor right away. If you also have chest pain, shortness of breath, back pain, numbness, weakness, a change in vision or trouble speaking, call 911.

Why morning and evening, two readings each?

The American Medical Association's guide for home monitoring asks for readings in the morning and evening, with two readings a minute apart each time, ideally for 7 days. The American Heart Association and the CDC also suggest measuring at the same times each day and taking at least two readings. This log is laid out exactly that way, so it is ready for what most doctors ask for.

Which layout should I choose?

Not medical advice. This log is a place to write down your readings. Your target, how often to measure and your treatment come from your doctor.

Questions

Is this blood pressure log really free?

Yes. There is no account and nothing to install. Print as many pages as you like.

Can I change the times of day?

Yes. Rename any time of day, add up to four, or remove the ones you do not need. You can also turn the pulse, time, notes and medication columns on or off.

Where do I write the top and bottom numbers?

Does it print on US Letter?
